If you run a specialist practice in South Africa, billing is rarely just “send the claim and wait”. Medical scheme rules change. Membership details are wrong at the counter. A theatre list is billed late because the admin team is already drowning. Short payments sit in the age analysis because nobody has time to chase them properly.
Outsourcing is not an admission that the practice has failed. For many specialists, it is the point at which medical account administration stops competing with clinical work.
1. Claims Leave the Practice Late, or Not at All
Medical aid claims in South Africa have timelines. If theatre notes, ICD-10 codes or assistant fees sit in a tray for days, you are already behind. Late submission is one of the simplest reasons practices wait longer than they should to get paid.
2. Rejections Keep Repeating for the Same Reasons
A few rejections are normal. The same rejection week after week is not. Typical repeats in specialist billing include inactive membership, missing pre-authorisation, incorrect modifiers, hospital versus rooms rates, and codes that the scheme no longer accepts.
3. The Age Analysis Is Getting Older, Not Smaller
Open the 90-day and 120-day columns. If those balances keep growing, credit control is not keeping pace with the diary. Medical debt collection for doctors is not the same as ordinary consumer collections. A dedicated credit controller who understands your speciality can work that queue effectively.
4. You Cannot See, in One Pack of Reports, What You Are Owed
Cash flow problems often start as reporting problems. If you cannot answer how much is sitting with schemes, how much is patient debt, what was rejected this month and what was collected, you are flying on gut feel.
5. Admin Staff Are Doing Clinical Work’s Leftover Hours
Many multi-disciplinary rooms still try to keep billing in-house because “we’ve always done it that way”. That works until volumes rise, a coder leaves, or the practice adds another specialist.
6. You Are Paying for Tools That Do Not Match South African Scheme Reality
Generic medical billing software, or a vendor that treats South Africa as an afterthought, tends to show itself in small, expensive ways. You need a local partner that already works with specialist practices across the country.
Synchramed has done this work since 2002. The service model is flexible: full bureau, account administration only, or software with support. A practical next step is a free practice assessment.
